(A) Candidate’s latest 5×5 cm. 3 copies of the attested color photograph in the size
(b) in favor of Deputy Commissioner, Gopalganj by depositing Rs. A 10.5×4.5 inch return envelope with a stamp of Rs.

Note that if the applicant is a freedom fighter/son/daughter/grandson/granddaughter of the martyred freedom fighter, then the certificate/certificate issued by the mayor/union council chairman to the effect that the applicant is the son/daughter/grandson/granddaughter of the freedom fighter/martyr freedom fighter.
(G) Attested copy of the certificate issued by the Deputy Director of the concerned District Social Service Office in the case of Physically Handicapped and Orphans.
